O.k I found a way to put the script in so that users can get the browse button up upload off of their hard drive. I was wanting to make something like this and was wondering how to go about doing it.
This link will show what to me looks like nothing more than a form that I can make.
But what I really want to know is how to get it to format everything like this once you submit your information.
