you don't need to use the rename/remove option just find the file on the LH side, hold the left mouse button down on it and drag it over

To Chris, the forms in question are four pages of detailed health history, I can't imagine trying to create them as fillable forms on line, but thanks for the idea.

Ah'Ha! The PDF is the best way then.
Its actually quite a problem trying to print a form the way i suggested if its more than one A4 page in length.
