You have added (somehow) an onClick event to your form, to the editboxes 1 , 2 and 3, the text area and the submit button. Also, your fields still have the original name (Editox1, Editbox2, Editbox3, TextArea1)

Name them with names like Name, email, Webaddress, Message.
