how do i add a form for submitting customer info

hello i am an insurance agent im trying to my a page wher people can input there personal info for an insurance quote!!! ANY HELP WOULD BE GREAT THANKS
after they input there info how do i recive the info???????

Depends on what you mean by SECURE...............

Any info from the form will only be emailed to you and the client
and not stored anywhere else.
