Contact us form not sending correctly

I just got done creating a form for people to email us at on our "contact us" page, but for some reason when i get the email notification, it doesn't display the message in the email. i sent myself a test message using the form, and in my inbox the email is from my own email address and it doesn't show any of the message, the email address i put in, name or anything else i filled out... it's just blank. I read the guide on how to create the form from scratch and made sure to follow the steps as best i could so i have no idea what could be wrong with it. any help in this matter would be highly appreciated.

I just sent a test email but have not received my copy of confirmation.
Also, on your nav buttons 2 pages are named "pageerror" ???

Oh don't worry about the other pages, I haven't published those yet.

Your message appears to have sent correctly and now the ones I sent from yesterday show up... that's very confusing to me because yesterday when I checked the test messages I sent they were completely blank... maybe there was a problem with yahoo and not the form? I can't think of a reason why they'd have been blank emails yesterday but NOW show up just fine ><
