How to add forms

Hi I just started using Blue Voda to build webpages and I am trying to add a form with the following headings:


Name:

Address:

Contact Phone Numbers:

Subjects:

Questions:

Age:

Form:

I would like to add these fields to a form and then add a submit button. When the submit button is clicked I would like all the info in these fields to be send to a particular email address.

I went to INSERT -> Form -> Form Area

But that is all I got so far. I don't know what to do from there.

Any ideas?
