Web Page Form Problem

Hello

My website URL is www.benefitplace.biz. I just created a new page where I created a form. The page that I published is: www.benefitplace.biz/linkedin-form.php. When visitors go to the webpage and fill out the form when they hit "Submit" the following message appears -"The specified value is invalid". What did I do wrong & what do I need to do?
