Website File Upload Problems

My name is David Dellinger. My website is http://www.holtsports.org/. When I upload the file for "Pre Register" it changes the text field for city and state on each page to "board" (a link on each website page). I can reload the index page which corrects the problem with the "board" in place of "Durham, North Carolina" on each page. However, when I do this and click on the "Pre Register" link some of the links on the left side are messed up see http://holtsports.org/forms.html.
