I have a product on my product page and i have installed a checkbox next to it from the forms page as well as a combo box with several options re rental duration but how do i then connect that information to a webpage that can process the information and then allow the customer to send it to me.??????

With a script... you really should look at MALS cart and get their codes to do this FOR you. You cant just create a cart without programming knowledge ... you need to USE tools already formatted or expect to PAY FOR IT. Nobody is going to sit here and create codes for your specific check out for free. See Mals Addon Cart or Cart manager.
