Bluevoda form page protection problem

I don't want members to sign up on my site.I rather want to create username and passwords to be used to open just a page<form page>i've tot about creating multiple accounts myself and removing register/sign up button/link but creating accounts with 1000 email myself is creazy.HELP.2 i also want completed forms to be previewed when members click submit for them to print their completed form
