VB form working with SOHO

Hi,

I have a form designed with VB and is working find in uploading a photo, http://www.tarjetasid.com/index.php?pr=Prueba. The form was placed in the web site via SOHO.

But when I access the same form through the following link, http://www.tarjetasid.com/media/Info...diantes.html I am receiving the following error messages after Submit,

What I am doing wrong?

Soho has its own built in form builder, you must use this, or link your form seperately, Soho will not permit the importation of a form.
