Has anyone used Form Maker Pro?

Can anyone who has used Fmp tell me if you can insert more the nine fields into a form I have

First Name
Last Name
Tel
Email
Preferred method of contact
Add 1
Add 2
City
State

And that is it! It won't even let me put a zip field in! Why? i need more fields than this it's a booking form!!

You can use Advanced BlueVoda Form Processor

It will process ALL and as many fields you like
