Problem with forms from my website

Hello,

I have strange problem. Normally, after the form is completed by visitor of my website, I am receiving completed form. Like this one, f.e.:

Values submitted from web site form :
Submit : Send order
check_agree :
email_testform : an@email.com.ua
name_surname_testform : Mrs. Eveline Lang
phone_testform : 8-050-419-50-87
Course_type : Standard
age : 18 and more years old
Payment_type : Stages
Localisation : Kyiv_centre
lessons_time : 19_00__20_50
Other_City : no

BUT, sometimes I am receiving something like that instead of completed form:

Values submitted from web site form :


And nothing more
Why it happens?
What a problem?

It is not because visitors are sending not completed forms. I have tryed. It should be another reason.

Just someone pressing the Submit button (to test your form) without filling in anything. If the form isn't filled, only the title (which is hardcoded in your script) gets to you.
