contact form assistance

I've got a mystery in my hands going on with the contact page. It was done to the letter as told in the instructions. Only thing that I could say was different was the uploading. Instead of first uploading contact page, "Thank you" page went up. Could this possibly be a reason why its not working??? http://www.iwittytv.com/contact.html

I'm afraid you have some errors in your form.

1. The php script, is in this page. It must be in the "action.php" page.
2. You have an "On click" event associated with your form. What is that for ?
