Email form not working

I thought I followed all of the directions in the tutorial, however, my email form, on my contact page, is not working at http://www.dpccrazycrafts.com and I am wondering if someone can help. I do know that I need to have an "action" page. Is this the "contact" page or the "thank you" page? I named the thank you page "action" as a php page. Should the contact page be an html page or does that, too, need to be a php page? I'd appreciate any help I could get!

1- Yes. The action.php page is the one that contains your "Thank You" message.
2-The contact page is the page that contains the form and can be named contact It can have a php or html extension.
3- The php script is pasted in the page properties>page html > Inside Body tag as per the tutorial

Now the only problem I see with the form is that the submit button is not on the form. Delete the submit button, stretch the form down a little and then place the submit button back on the form. Save and publish.
