my submit button on my form brings up message 404-cannot be found

ive been over the forms tutorial many times now and did it the way they show, but on my site when i hit the submit button i get the ''cannot be found'' message. any ideas what ive done wrong?

i did notice it submits, and your ty page happens before i read your second post on this thread, just i didnt think you could submit with out a valid email, maybe need to name e-mail as e-mail too?
