When you say it isn't working what is happening, does the form submit successfully? I can't read your site to figure out how far it gets...

You have set the form action to be "action.php" however no such page is found. So i suppose that your thank you page (action page) is saved with some other name, or is not published at all. Check it, and make your form action be excactly the name of the thank you page.Navaldesign
