Quiz Form?

So I am trying to create a relatively interactive outdoor site. I would like to be able to add a picture of a animal species and then have some options to choose from.. It then pops up to a screen that says congrats you were correct or sorry incorrect with the correct answer for people to view for their information. I also want to be able to add one big quiz with like 10 questions and then press submit and it ends up on a page that tells scores and what questions were right and which were wrong. How would I go about doing this? I would rather keep it down to where it's all my site no scripts with linking off to other sites etc... How could I do this?
