contact form not working

hello; i have tried to follow all the directions, checked it once and then again. i am getting this url when i press the submit button instead of my thank you page:

please tell what i have done wrong

Problem nr1: the hidden field is named "formid" instead of "Formid" (uppercase F)
Nr. 2: ABVFP will limit your comments field to 1000 chrs, not 1000 words
Nr. 3: if you want - in the phone number, your field should NOT be type NR but RA

Address should better be a text area, to allow users seeing all 3 or 4 lines while typing

The main problem is the F (Formid)
