I have been trying to complete a form and thank you page as per the video but I do noy get the html script as shown on the video . the nearest I get is
mailto:http://www.artinwax.com/thank_you.html

I assume I alter this to my email address.
I cannot see mailsubj on the page.

Your thank you page has to have the extension .php, not .html, because it is the action page for your form. You will have to delete it from file manager and republish with the .php extension.

you will also need to fix your form properties...doubleclick on your form and in the action box put thank_you.php

Hopefully these two steps will fix your problem. You will need to republish both pages.
