Form to insert text in document

I need a little help to produce a form that will insert text into a document. The form will be used in a web page or in an ebook format. In the input part of the form - its a matter of simply typing in information in a text box. Upon clicking submit, the form then needs to insert the typed information into a sentence or paragraph in a document. An example can be found at this web site

where a person types in information for a will and then the form inserts the information in a document form. Can this be done with the BV form builder or does it require a special script of some type?

Any help would be appreciated.
