new form prob.

This is my first form on my website http:www.ivracing.net and and had no problem building the form I used the wizzard,and seemed that everything was good ,I saved & published it in php ,verified the link ,but just wont come up while online,I can type in the address in the browser http:www;ivracing.net/onlineentry.php and it comes up,but when you submit, it just goes into an error,mabe someone can check it out and see what I'm doing wrong ,,,please....Thanks...Opendezr****
