Problem with form page, Please Help?

Hi.

I just updated my site with a new design but my contact page has messed up for some reason, the header and buttons have moved down

As said in the video tutorial i have saved the page as a php

Does anybody know why this is happening?

Stupid me, i made a stupid mistake, my link in the button was .html and the page was .php
