Form Wizard - email content not correct

I've made a form wizard on my contact page and have published it. When I send a text message off the internet to my email address all that comes up in the email is "Customer Enquiry" whih is what I had called the form. None of the other infomation that I had typed in the boxes provided cam up. What have I done wrong? If someone were to try and contact me through my form wizard I wont know who it is and what they have said and will loose business. Please advice.

Dont blame the form if you lose business, blame yourself for not taking the time to view the vodahost tutorials 21 - 22 - 23

Every thing about your form is wrong:

fields are not named
action field is wrong
