Linking form to index - please help me

I am a beginner but have managed to create and publish my index page. I have also managed to link another page to the home page.

At the moment I am at a complete loss as to how I can link a page containing a form. I have watched the tutorials so I understand how to create a form. This I have done. I have also save the page containing the form and I have published that page.

How do I link it?

After saving and publishing the page I have gone back to create the link but the link icon will not activate - what's with that?

Can someone please give me a step by step answer - please don't tell me to watch the tutorial becaise I have done so.

You link the page with the form the same as any of your other pages. What did you save the page as? (ie: contact_us, etc) It does not matter that a form is on the page or not, they all must be saved and published the same. The only difference is getting the form itself to work correctly, thus the videos.
