Problem after publishing form

Hello, I published two forms www.quatre-feuilles.info/reservation.html and www.quatre-feuilles.info/contact.html and I see a lot of text above the form after publishing. Thanks for your help.

All pages with forms on them have to be published as php, change the flie types on both pages, save and republish. You will also need to change your links.
