Form gives error message

Hey.
Please checkout my form on the top left on this site.
It gives this weird error message. I posted this problem a few days ago but the answers I got didn't fix the problem - I checked out the success and error pages - they're OK.
Furthermore - this form is a "copy paste" of another form on my site which does work - meaning - I didn't change anything (except for the fact that this form appears on every page as a template) and than it stopped working.
I am receiving the contact information though I am getting an error message.

You can not copy and paste a form from one page to the next, you need to create the form again on the page or clone the page with the working form. Copy and pasting anything in BV is not a good idea, most of the time it will not work.

Hey.
Thanks but that still didn't help.
Please take a look at the error message. I think it has something to do with the fact that the form is place on the template that goes for all of the pages.

You have used the frame set coding.................

This will work if you put the url of the form in a BV Iframe.

The demo was simply to demonstrate that your form would work in a BV Iframe instead of your method of using frame sets..................

So what are you saying exactly ?
To put the form inside a frame ?
What difference will that make ?

Try it and see, and if you still get that eror message, then change that menu.php

I did a quick simple demo just to prove that it could be done, mine works...yours dont.....nuff said.

Yes, the problem is that the form page is included AFTER the page is loaded (and sent output to the browser) so, after the submission, when it tries to redirect to the thankyou page (using the php header() command), it encounters this problem because headers are already sent out.
