form page & success page

i have a form on the link easter exhibition page but i cannot get the link to the page to work or the form amd sucess page to work, i have done lots of pages but cannot see my errors here, any help is much appreciated. many thanks

just put
success.html
at the bottom and nothing else

you may want to name your fields at the same time
