index.php problem

I have several forms on my website that are working properly. But I am not receiving one from my home page form.

I'm sure it's something minute but I just cannot figure it out. Any help is appreciated. www.pahomesave.com

You have both an index.html and index.php page. Use FTP Manager to delete the html version--the net will always look for that one first when you click on your url.
