Need help with Form Maker Pro

I try to go to my web browser and type in www.mydomain.com/fmp/install.php and i get

Template Error: set_root: tpl is not a directory.
Halted.

Can any one help me?

Not all files have been uploaded, some folders or subfolders or files are missing. Check it.
