Can't find my Form Maker Pro anymore

I recently updated my Firefox browser and lost all my bookmarks. Now I can't find my FMP account or what URL it is at. Did FMP change? I find a site that looks similar but it does not recognize my username/pass.

I don't have my treasure chest pass anymore as I downloaded files two years ago or so.

Oh my gosh, Spotlight just found an obscure file with my info on it.
