Drama publishing Form on Index page...

Hi guys...I have spent several hours searching the forum, and viewing the video tutorials in search of an answer to my problem. Please excuse me if I'm missing something simple( as I'm sure it is)...but I'm out of patience!

I've published my site index page(php), but my form is not visible.

I've published the form page(php...which by the way works fine)

I've published a success page and also an error page...which are both fine.

Please...please...could someone help out a complete newbie at this...never published a Form before...but once I know what simple thing I'm missing, I'm away.

I had a thought I might be linking it wrong...but I'm not even sure how to link it...could someone please explain...I've done the tutorials...having a brain meltdown now.

    Cheers.

At some point youve published an index.html which takes precedence over your later published page index.php. Login to control panel ... file manager and delete the index.html

as you can see the form is working here

    but missing from

just make sure your index stays as php and dont change it up to html. Other pages can be either as long as they dont have a form on them or require php.. thats up to you.
