User Registration Form

Hi fellow BV designers,

I'm busy with a freebie site and would like to know if you can point me in the right direction for creating a registration form for new users on the site. Should be database driven as registered users must log in. The new users must also be created automatically.

Thanks George,

Looks good. This is what I think will be good for my friend's website, but I will first have to talk to him and see if he wants this and willing to carry the costs. I do the rest of the site for free (design, domain, hosting costs for at least 1 year). (It is not the site in my signature).
I will still enquire on your form about the second option.
