Form Problem

Visitors to my site have complained that my form sometimes doesn't work. Why would it work sometimes but not the next?

I got no idea how to fix something I don't know about. So what must I do?

The form I am talking about is under the 'advertise here(accommodation)' link.

Maybe you can try it out and see what it says and we can go from there?
