form does not appear when published
I have a problem with my contact page. I have a simple form and have created success and error pages. However, when it is published it does not appear. I have tried everything and nothing!

Not knowing as you have not posted a link to the page, but at a guess one of to things to check.
1/ when you publish a page with a form on it will publish as php and not HTML so check the link, making sure it ends with php like contact.php
2/ when publishing make sure that the box on the pop starts with public_html/ -

I have published it php and on public.html, so I do not understand. Any idea of what could be going on? Also when I publish it as html instead of php it shows up, but with lots of writing on the top.Comment
