form Maker Pro

Hi, i just installed Form Maker Pro last night.
i'v made a form with 2 pages.
I got my pages, uploaded them to my server.
i click the link for the first page of my form, filled in the information and click the next button and get this error Interal Error 500 plus a 404 error.
What step am i missing that i can't get to my second page??

Hi i have install Form Maker Pro, io have made a form for customize calendar,the form is not finish but i have 1 page and i have Cover,January,February and March to test it. Everything works at it should but i have on problem, the picture for the month of March doesn't show up in my email, all other does.
I was wondering if there was a file limite and (or) attachments limite? And if so what file would it be in? Is it changable?
