    adding form to website correctly

    www.Lwalkercycling.com/team2 (location of the form)

I am not adding this form in my website correctly, please help.

Trainingpeaks.com sent me their form instructions. I opened the form whizard on BV added their (TP) instructions to the form actions, published and can see the form. When I enter my password, as a test, I am taken to a junk page.

Am I using the correct BV form? If not, what do you suggest.

If I am using the correct BV form - what steps am I missing?

    Thank you in advance.

    Is this it http://www.lwalkercycling.com/team2.php

Note the .php extension. When using forms with BV, the page property is automatically changed to .php so it will work,

Well, you have made a mistake: the code they provided you ALREADY is enough to create the form. You should not have added a BV form.
        Delete the entire form
Add a HTML box and paste the code in it. Publish. Refresh your browser to test.

An Html box - are you referring to "View" / Page HTML?

Main menu - Insert>Advanced>HTML D/Click on the html box when on the page & paste your code, press ok. Save, publish.
Might have to be published to fully work.

Will work also in preview as it is simple HTML code. For the same reason the page needs not be .php, although you can also have it as .php.
