2 form fields not working

Hello - I am having a little trouble with my form:
it seems to work fine except when I check the e-mail from the form the first 2 fields don't show up, name and address. I have read all the posts here and checked the form properties to make sure the correct boxes were unchecked, and that they were inside the form boundries. I can't seem to locate the problem.

I see those two fields ok. Maybe I'm misunderstanding the problem.

I just deleted it and started over. Seems OK now. Thanks for looking!
