I just figured it out! Just wanted to share in case anyone else is having this problem. I followed the instructions to install form maker pro. Then I went to the http:yourdomain.com/fmp/install.php and I got an Error Template message. So I searched the forums and came across one on this subject and it said that BlueFTP has a bad habit of not transfering ALL files and folders over and to remove it and try again. Well, I tried and I tried but couldnt' get it to work. So.....I logged into the FTP manager once more and opened the "fmp" folder on the left side of the screen and opened the "fmp" folder on the right side of the screen. I began to go through and compare each file and folder to ensure that each side had the same files and folders. Sure enough, I found that two were missing so I highlighted the two that were missing from the right side. I highlighted the two that were missing and dragged them over to the right side. VIOLA!
