Multiple page form

I am creating a multiple page form. There are three questions on page 1 and the answer to the third question on page 1 on "Continue" will determine if user goes to page 2a or page 2b. If the user goes to page 2b, the last question will determine if user goes to 3a or 3b.

Can you tell me how to do this? I do not code but can copy and paste with the best of them.
