Form with subscription

Hi guys, i wanted to know how if it is possible to make a form where they submit and subscribe at the same time.

So all their details get sent to my by email, while they have also subscibed for a monthly payment for a service.

So i can know who im helping, and what payment plan (subscription cost) they are on.
