    Form not submitting to mailbox

    i created a simple basic forms and a combobox / drop down list to show the . However i uploaded (published ) the form and when i tried to test it gave me an error message . I have uploaded some screenshots please i would like to know whats wrong.

      just to make it clear i created the orderthanks.html and error.html pages also

          I have some probleme to you, do you find the solution because i coming crazy with blue voda nothing work.
