addon domain form not submitting

My main domain is www.bestintentionsfarm.com. I have a form for camp on this site that works fine.
My addon domain is www.reconnect-relationships.com. I have a form on this one that is not sending to my email correctly,
"HTTP 404 Not Found", I believe I have set up my error and success pages correctly for this form (other one works great).

Not sure what I've missed.

what page did you put the form on?

campform.bvp, I used the new Form Wizzard (always done it the "old" way before) and checked, the page property is a .php but the form itself is .bvp.

www.reconnect-relationships.com/campform.html

Form should submit to inforeconnectrelationships******.com

success and error pages are created.
