The correct terminology would be "unblocked" rather than whitelisted.

It may happen again if:

1. You try to login with wrong details, more than 3 times
2. Someone attacks the server (or, in anyway, activates a "suspicious" connection) from the same IP address as yours. It could be anyone that uses the same ISP that you use
