BVAFP Blank Error Box

I need some help with a form I created. The form is here:

Anytime required info is omitted from the form the error page pops up but the box is blank. I found this thread for a very similar issue and tried the suggestions but it did not seem to help.

What am I missing?

I figured it out. The tutorial said to use their error page until becoming more familiar with the scripting. I had originally just copied the content on the provided error page onto my error page. I replaced my error page with the one provided then it worked. I then modified the provided error page to fit my website. So there is something on the provided error page that needs to be there...

Yes, there is code (php code) in the page. This is explained in the manual.

However, the 404 error is a "Page not found" error, so I can suppose that you had simply mistaken the URLs.
