Reused form not aloowing data imput

I have re-vampted an old form and it seems as though now no data can be inputted to it on the web. Any ideas what I can do...except for rebuilding the whole thing again! The form is found on www.mowcopchurch.com/bugs_holiday_club.php

Sorted it! My text boxes from my labels were covering the edit boxes!!
