Support Form with unique number

Hi,

can you please tell me how can I make a form to my support page, on which I can receive and inform my clients their unique support number?

The idea is to make something like the support tickets from vodahost where I can have for example:

Ticket Details
Ticket ID: JYO-525958
Department: VodaHost Billing / Accounting
Priority: High
Status: Closed

Why not simply install a fully-featured "Support Ticket" script on your site?
There are a few to choose from to auto-install via the Fantastico section in your cPanel .... they have the same features and functionality you are particularly looking for.
