how do u pass name and email field from 1 form to another form

hi,

i'm using form wizard on both of my site.

i'm trying to pass the name and email from site 1(optin page) to site 2(survey page).

the reason for another optin page on site 2...
is so i can see who the info/data submited came from which email address.

1)how would i go about passing name and email from 1 form to another form?

or...

2) mayb how would u do it with without having to submit email again.
for example the 2nd page would just have the survey options to choose from.

but when they submit the data.
their name and email would be hidden in the background.
