check box and submit button

How do I set up check boxes and a submit button:

1. Check boxes for several options.
2. Set up a submit button at bottom of page.
3. Customer select the option, click submit button and it goes to a form page for name and email address info.
4. Click submit on form and it goes to my email address so I can be notified.

How to set it up by using the different functions? Please advise.
