satisfaction questionaire

Hi, I built a form (satisfaction questionnary for my clients)
It works, I arranged the order of items but when I receive the form by email, I see that all items are written in the same line. I would like to see each item line by line to have a better overview. How can I fix that? Thanks for your help

Hi, can you please submit a support ticket regarding this, we are going to need to look at the emails coming in themselves, so this needs to be handled non public!
